Annual Access Membership Plan
$2400
Dr. Godwin provides a membership plan for individuals seeking a
readily accessible personalized health advisor. Members will benefit from an in-depth initial consultation as well as up to 6 additional visits with Dr. Godwin. Members also receive access to Dr. Godwin’s cell phone and email for health-related concerns, both urgent and non-urgent, throughout the year. New visit fees may be used toward the annual membership if clients wish to upgrade after seeing Dr. Godwin.
Dr. Godwin suggests membership for individuals in one or more of the following circumstances: Clients working closely with Dr. Godwin toward personal physical and/or mental health goals. Clients who require prescription refills for weight loss medications or controlled substances. Clients who seek a readily available physician advisor for acute care concerns, care on demand while traveling, or expedited attention for prescription refills.

What conditions can Dr. Godwin address at my initial consult?
Conditions addressed during this initial consultation include, but are not limited to weight management, attention deficit disorder evaluation, anxiety, depression, sleep issues, fatigue, brain-fog, hormone function concerns, pre/peri/menopause, lack of motivation, chronic pain, headaches, digestive concerns.
How will insurance be incorporated into your cost for care?
Appointments with Dr. Godwin are billed directly to clients. Health savings accounts may be used. Patients are welcome to request a superbill to submit to insurance, however, Dr. Godwin cannot guarantee reimbursement.
Insurance is recommended as Dr. Godwin will directly bill all labs, imaging, and prescriptions to your insurance provider. PPO insurance plans work best for optimal coverage.
